Úvod
 

11th Revision of the International Classification of Diseases, MKN-11  

      Dne 18. 6. 2018 Světová zdravotnická organizace (WHO) vydala 11. revizi mezinárodní klasifikace nemocí (11th Revision of the International Classification of Diseases, ICD-11, MKN-11).

      První vydání mezinárodní klasifikace nemocí, známé jako Mezinárodní seznam příčin úmrtí, bylo přijato Mezinárodním statistickým institutem v roce 1893. WHO v roce 1948 zveřejnila 6. verzi ICD-6, která poprvé začlenila nejen příčiny mortality, ale i morbiditu. Nomenklatura WHO, přijatá v roce 1967, stanovila, aby členské státy pro ukazatele zdravotního stavu používaly nejnovější revizi ICD. ICD byla revidována a publikována v řadě edic, aby v průběhu času odrážela vývoj ve zdravotnictví a pokrok v lékařské vědě.

      ICD-10 (MKN-10) byla schválena v květnu 1990 na 43. světovém zdravotním shromáždění (43th World Health Assembly). Je citována ve více než 20 000 vědeckých publikacích a používá se ve více než 100 zemích světa.

      Poslední verze - ICD-11 - byla zveřejněna minulý týden, dne 18. června 2018, aby se členským státům umožnilo připravit se na implementaci, včetně překládání ICD do svých národních jazyků. ICD-11 bude předložena na 144. zasedání Výkonné rady v lednu roku 2019 a na 72. světovém zdravotním shromáždění v květnu 2019. Po schválení začnou členské státy využívat ICD-11 pro účely hlášení nemocí a statistiky nemocnosti a úmrtnosti od 1. ledna 2022.

Nedostatky a problematická místa

     Zdaleka ne vše novější bývá lepší. Ani nový klasifikační systém nezůstal bez vad. Osobně považuji systém písmene a číselného kódu o dvou číslech a třetím za desetinnou tečkou v ICS-10 (MKN-10) za přehlednější. Navíc mnohá písmena napovídala oborovou příslušnost nemoci, např. C - cancer, M - musculoskeletal disorders, N - nephrology, O - obstetrics, P - perinatology, T - traumatic causes atd. Překvapilo mě, že se neodstranily některé zřejmé nedostatky MKN-10. Dodatečně upozorňuji na některá problematická místa:

  • není jasná hierarchie některých nadřazených pojmů - kde končí nadepsaný oddíl, proto jej sám doplňuji ukončovacími značkami (- - -)
  • zmatený systém neuroinfekcí - např. virové neuroinfekce v oddíle "non-viral infections"
  • doslova tápání v malých a velkých písmenech - kde nebylo nutné uvádět velké písmeno u jmen virů, zavádím malé písmeno
  • chybění kurzívy v mikrobiologických druhových jménech, které jsem doplnil
  • totální rezignace na latinu - pak se objevují názvy jako Whooping cough, Hookworm diseases, zavádějící je typhus fever (pro skvrnitý tyfus) a mnoho dalších
  • mnohde se klasifikace jeví nerovnoměrná - např. dengue jako téma nejvyšší úrovně
  • nepraktická snaha o postižení kombinací - např. malárie a infekce HIV (to už by se musely řešit všelijaké interní komorbidity a dispozice); těžkopádná je např. kombinace "1C61.30 Kaposi sarcoma associated with human immunodeficiency virus disease associated with malaria"
  • uvádění diagnóz s pochybnou existencí, jako varicelová meningitida, do hlavního výčtu, což při nezkušenosti kódujícího lékaře předurčuje omyl (klasifikování encefalitidy jako meningitidy)
  • zvláštní systematika streptokokových infekcí; není jasné, zda má být uváděná "1B53 Meningitis due to Streptococcus" i jako diagnóza pneumokokové meningitidy
